A group of drugs called phosphodiesterase-5 inhibitors has revolutionized the treatment of erectile dysfunction. The three main drugs in the group, sildenafil (Viagra), vardenafil (Levitra), and tadalafil (Cialis), have become household names. They are heavily advertised during televised sporting events like football and baseball!

These drugs all act in the same way but vary in how quickly they begin to work and how long the effect lasts. They help the blood vessel supplying blood to the penis to open up, allowing blood in faster than it drains out, and so the penis becomes stiffer for longer.
Because they cause the blood vessels to dilate, they can cause headaches, flushing, and runny nose as side effects. If you have heart disease and are taking nitrates, they may not be safe. They can sometimes cause problems with your vision, and if they work too well, can cause the penis to stay so stiff and engorged for hours on end that it can cause pain and damage to the penis (this condition is
called priapism).

All of these drugs work well for most men with diabetes who have erectile dysfunction. If they do not work, or if you are not able to take them because of side effects, then there are other options. An injection of papaverine or phentolamine or alprostadil given into the base of the penis can cause an erection sufficient to allow intercourse. This is obviously a bit more painful and awkward than simply taking a pill but can be quite effective. Sometimes two of the drugs can be used together. Alprostadil can also be given by pushing a thin tube up the penis and injecting the drug from inside.

More mechanical solutions include vacuum devices to pull more blood into the penis until it is enlarged and stiff enough to allow intercourse. This is followed by placing a rubber band around the base of the penis to keep the penis stiff during intercourse. If the erectile dysfunction is due to a poor blood supply to the penis, then surgery to improve the blood supply can be successful. Plastic implants can also be inserted inside the penis to keep it stiff enough for intercourse.

Since the introduction of the phosphodiesterase-5 inhibitors a few years ago, it is much less common to need to use the alternative treatment that I have described. Your doctor can go over these options in more detail or can refer you to a sexual dysfunction specialist.

Not at all, especially if you can keep your blood glucose levels close to normal, eat a healthy diet, and avoid drinking alcohol to excess. But over half of all men over fifty with diabetes experience some degree of erectile dysfunction. Even though they have interest in sex and are in a healthy, happy relationship, their erection becomes gradually softer and does not stay hard for as long as it used to. There are a lot of reasons for this. A normal erection requires lots of things to work well. You need to feel relaxed and unstressed. The nerves supplying the penis need to be healthy, and the blood supply to the penis needs to work well. If you are stressed and worried that you might not be able to get an erection, that can upset the process. This is sometimes called performance anxiety, because worrying that you won’t be able to get an erection that is stiff enough to allow you to perform sexual intercourse can turn a slight problem into a major one very quickly.
If the nerves are damaged from diabetes, they may not give the correct signals to the blood vessels to let more blood into your penis and prevent blood from leaking out again. And if you have narrowing of the arteries supplying your penis, then not enough blood can get through to cause an erection. Any combination of these problems can cause erectile dysfunction. Hormone problems such as low testosterone levels or high prolactin levels can also occur, but these are not a common cause of erectile dysfunction in men with diabetes. Your doctor should be able to figure out what the main problem is for you by talking to you, examining you, and doing a few simple laboratory tests.

This can be awkward and embarrassing and difficult, even if you have a good relationship and are good at communicating with your spouse or partner. You can certainly ask your partner to come with you to a doctor’s appointment and ask your doctor to explain it to both of you.

Here are some of the things that you should explain to your partner. Your erectile dysfunction is not your partner’s fault. You still find your partner attractive. It is not your fault either. You are no less of a man because you have erectile dysfunction. Usually erectile dysfunction is not caused by a low testosterone level in men with diabetes (but your doctor will check to make sure). It is usually caused because the delicate nerves that cause blood to flow into your penis to make it stiffer have become damaged from diabetes. There are several treatments that can help. If you feel tense and under pressure to perform, your erectile dysfunction will get worse, so if you and your partner stay relaxed and good humored about it, things will go better. If you find other ways to give each other sensual pleasure without being under pressure to have sexual intercourse every time, this will help. Counselors at sexual dysfunction clinics often have specific techniques for you to try that can really help.

Dear Doctor I am 30 year old male from the Philippines.I had a HIV rapid test
done with me 5 weeks ago.My question is regarding proper blood collection.I had
a finger prick I knew the lancet is new my concern is a micropipette with
replaceable tip was used to get blood on my finger. The tip was also new for
what
I know. But the thing that scared me that someone had been found reactive before
I had the test.I learned about it while waiting for my result and talking with the
nurse
who took my blood she even showed me the assay kit that was used.Surely the same procedure
was
performed and the same micropipette was used with that person except the tip
and
the lancet.I had a non reactive on that test I was very happy, supposed to be
my
conclusive 3 months. When I got home searched about the test kit there Ive seen
the instruction that micropipette was supposed to use in extracting serum not
whole blood on the finger.A capillary tube should be used on finger prick. My
question is that is is proper and safe to use such device in a finger prick
procedure?Do I have another risk?Could you please enlighten me on this
matter.Thank you so much
