Do hypoglycemic symptoms occur at the same blood glucose level every time?

No, they don’t. People vary a lot it in what symptoms they get and what blood glucose level they need to be at to get those symptoms.
If you have had high blood glucose averages (let’s say over 200 mg/dL, or 11.1 mmol/L) for a long time, you might start feeling hypoglycemic symptoms when your blood glucose falls below 100
mg/dL (5.6 mmol/L). But if your body is more used to lower blood
glucose levels and your average blood glucose is 120 mg/dL (6.7
mmol/L), then you may not feel hypoglycemic symptoms until your
blood glucose is less than 60 mg/dL (3.3 mmol/L) or even lower.
Symptoms sometimes come on at higher levels if your blood glucose is dropping fast. If your blood glucose is dropping very slowly, it can sometimes get a lot lower before you realize you are low and symptoms start. If you are wide awake and alert, you will tend to notice your hypoglycemic symptoms at higher blood glucose levels than you will if you are sleepy or distracted. People who drink coffee or other caffeinated products often notice their hypoglycemic symptoms at higher blood glucose levels.
